Foster Agreement Contract:
 

The foster family fully understand that they are to act as a foster home for one or more of the Earth Services animals, referred herein as "the animal" or "the animals." The following conditions apply to fostering of Earth Services animals while in the fosterer care and in their home:

The animal will be kept in the house or in a fenced yard on the grounds of the foster home at all times. Supervision is necessary. At night, all animals will be provided with shelter.

The animal will remain the property of Earth Services. The foster family is unable to adopt the animal out, move the animal, or adopt the animal unless it is done through Earth Services' central office.

The animal will be fed, watered and exercised appropriately.

No prong or shock collars are to be used at anytime for the animal.

The foster home will not be compensated for expenses incurred in the normal daily care (food, water, transportation) of the animal. Earth Services will be responsible for health and medical care, but only when agreed to by Earth Services prior to treatment. The foster family will incur charges if not cleared by Earth Services.

Earth Services is responsible for vaccinations, heartworm testing and preventative (if applicable), altering and any prescription medications if necessary for the animal.

The animal will be adopted to it's permanent home only under the supervision of Earth Services to a family pre-approved by Earth Services. If Earth Services requests it, adoptive families may be contacted by the foster family to make inquiries about the animal, however the name of the adoptive family and information is to be kept private. All contacts are to be done with professionalism. All decisions regarding adoptions are to be made by Earth Services central office.

All donations connected with the adoption of the animal will be sent directly to Earth Services. Questions or problems are to be directed to Earth Services.

The foster family is to accept responsibility for making arrangements to get all animals to events.

Earth Services is not responsible for any and all events that occur in connection with the fostering of a rescue animal. The foster family agrees to release Earth Services from any and all claims, know and unknown, in connection with the animal.

The foster family will evaluate the animal and prepare an evaluation form. This form will be kept with the adoption package as information for the new owners.
